The datasheets are optional tools that have been developed to assist you in your in-store data collection. Should you choose to use the datasheets, they can be easily copied for use in email or note-taking apps.

We strongly recommend Evernote as a tool for in-store data collection. Evernote will work both online and offline.

Sample Surveys

The following is a practice survey for you to review ahead of your store visit. Feel free to enter data as you go through the survey to practice ahead of your visit. Please take note of the instructions detailed above each section of the survey which will outline exactly what data to enter, including when to enter a '0' vs. when to leave a field blank if unobserved.
